Sinan Haddad, MD has been designated as a Civil Surgeon by the U.S. Citizenship and Immigration Service (USCIS) and now performs affordable, quality immigration physicals. Please call us at (678) 680-4290 to schedule an appointment.
The physical generally takes 2 visits to complete and our examinations are $350 for Adults and children. This includes both visits, Gonorrhea Urine Test and Blood Work for syphilis and TB, additional charges may apply if more testing is required, such as x-rays. Payment is requested at time of service.
We also offer the following immunizations for an additional fee of $85 and up per vaccine:
- Tetanus
- MMR
- Hepatitis B
- Influenza – only if in season (October 1st – April 1st)
Immigration Physical Checklist
- Proper identification (at least one form MUST be a photo ID)
- Passport
- Driver’s License
- Green Card
- Birth Certificate
- State Issued ID
- Military ID Card
- Alien Registration Number (if assigned already)
- All necessary paperwork
- Medical records (translated to English, if necessary)
- Immunization records (translated to English, if necessary)
- Form I-693
North Georgia Urgent Care will keep your immigration physical paperwork until all tests have been completed. It will then be released to you in a sealed envelope. Do not open the envelope, as it is required to be presented to the USCIS sealed and unopened. You will be provided a copy of your physical free of charge.
Dr. Haddad is not responsible for determining whether an applicant qualifies for an adjustment in status. That determination is made by a USCIS Officer after reviewing all relevant records, including the report of the medical examination.
